size() and save sketch problems

I have some occasional problems with sketches not being saved correctly, or missing some of the updates since last save.. usually involves the sub-files, not the main-sketch file..
and on some sketches i'm having real problems getting size and smooth to work..

when i press run it just opens the defult 100x100 window without smooth(), and 3/4 times when i get the correct size, smooth() won't work.
But when it finally does work, it works till i've made some radical changes it seems..


any ideas?

please use separate threads for separate bugs.
and please give some actual background on what's going on.
