Are you starting with an image in which you have to detect the QR code, which could be rotated or at an angle?
This is a horribly nasty problem! You'd have to do a ton of image filtering! This would be a nightmare!
Are you starting with a plain QR code in an image?
You'd have to do edge detection to locate the black and white squares. This is a tricky problem!
Are you starting with an exact bitmap of a QR code where each pixel is either black or white?
